​Available 24 hours a day, 7 days a week, the Generation Hospice team works closely with patient and family to develop and coordinate an individual plan of care. Each member of the hospice team is specially trained in hospice care.

​Team Members include:   

The patient's Primary Care Physician and the Hospice Medical Director , who work together to direct and guide the patient's care to ensure the goals are being met.

Registered Nurses who coordinate all services for the patient including assessments, teaching, administering medications and evaluating the disease process. They offer emotional support to the patient and family. Their primary goal is to palliate or relieve any distressful symptoms that might arise as the disease progresses.

Social Workers focus on the goals, needs, and strengths of both the patient and family caregiver. They provide counseling, help navigating the healthcare system, facilitating advance care planning, intervening in crisis, mediating conflicts within families, and linking clients with resources.

Hospice Aides who provide personal care and assistance with activities of daily living. 

Spiritual Counselors who provide spiritual care as requested and/or deemed appropriate through the patient's religious affiliation.

Volunteers who are available to provide companionship and other services to the patient, caregiver, and family.
